A New Year, Newfangled COVID-19 Variant B.1.640.2 (IHU): What We Know So Far?

Omicron, which was first discovered in November of last year, was only just beginning to be understood when another strain uncovered in France made headlines. On January 4, 2022, news of the variant exploded on social media, but cases of what is now known as variant B.1.640.2 (IHU) were initially discovered about two months prior. Evidence is still being gathered, but internet misinformation regarding the latest coronavirus variety is already rampant, as it was with Omicron. The majority of existing vaccines target SARS-spike CoV-2's protein, which the virus utilizes to enter and infect cells. Epidemiologists and virologists worldwide are concerned about the virus' spike protein, which plays a key role in how your body identifies and reacts to the virus. Spike proteins are produced, recognized, and defended against by our immune system. Your body and the vaccines you have had injected into your system have a far harder time fighting the virus when the amino acids in a protein are altered or removed.

Leprosy Reactions: Clinical Pharmacologist Perspective with Repurposed Medications
The elimination of leprosy has been possible with the available anti-leprotic drugs. However, the lepra reactions usually occur months or years after multi-drug therapy completion, and continue to be a formidable challenge mainly owing to its role in causing nerve damage and disability. Corticosteroids are commonly used but they lead to systemic complications, and hence require dose reduction and adjunct therapy with a different target. Various drugs with different targets have been identified and are in practice to treat lepra reactions. The newer targets can include genetic and tissue targets in the skin and nerve. Thalidomide treatment reducing pentraxin-3, toll-like receptor antagonists, minocycline, apremilast, immunomodulators, and tenidap can be helpful in lepra reaction. Other modalities to manage lepra reactions include plasma exchange, intravenous immunoglobulins, and immunotherapy. Most of these treatments are based only on the pathological process of the reaction and tend to be incomplete leading to recurrence. Newer multimodal approaches are required based on various biomarkers (genetic, tissue, serological), which can be monitored to prevent the recurrence of reactions. Hence, there is a need for newer targets and drugs to be identified for the management of lepra reactions.

Management of Denture Infection with Phytomedicine

Absence of dentition subjects the individual to compromised oral function, facial appearance and phonetics. Rehabilitation with dental prosthesis is imperative to restore form and masticatory activity. Currently, fabrication of removable and fixed prosthesis has become most predictable. Nonetheless, there is an increased prevalence of oral stomatitis observed over the years in edentulous individuals wearing removable dentures. Amongst the many pathogens that cause denture infections, Candida albicans is assumed to be the most virulent opportunistic agent. Different methods, such as mechanical, chemical, chemicmechanical, and pharmacological are tried to fight such infections. Regardless of various management strategies developed to treat denture stomatitis, the research continues to evolve the most optimal one. Improper oral and or denture hygiene maintenance, surface irregularities on denture, persistence of xerostomia and associated systemic illness pose risk for exaggeration of the disease. In extreme conditions, the development of aspiration pneumonia in geriatric patients is considered a threat to both dental and medical specialists. Therapeutic administration of synthetic anti-microbial drugs, along with meticulous oral hygiene maintenance are recommended protocols till date. However, limitations such as antibiotic resistance, side effects, counter drug reactions, cost of the medicaments predispose to origin of natural herbal products to treat denture stomatitis. Vast array of plant products are studied in previous literature, yet no definitive edge of one over the other is proven. This article intends to provide a mini-review on the different organic plant materials, also called as phytomedicines used for the treatment of candida associated denture stomatitis (CADS).

Identification of Virulence Markers and Phylogenetic Groups’ Association, and Antimicrobial Susceptibility of Uropathogenic Escherichia coli Isolates
Background: Urinary tract infections represent a world public health problem, which is caused mainly by Uropathogenic Escherichia coli. Although they are originally found in the intestinal microbiota in the majority of the cases, urinary tract infections can also be caused by intra-intestinal pathogenic E. coli. Objective: The main objective of our research is to identify the virulence factors generally associated with different pathotypes across phylogenetic groups. Methods: E. coli were isolated from patients with urinary tract infections. Antimicrobial susceptibility tests, virulence genes and phylogroups were prospected. The data analysis were performed using the chi-square and Fisher exact test. Results: In total, 72.2% of isolates showed multidrug resistant. We have also depicted an important association between E. coli from inpatients with UTIs and pap and hlyA genes (p-0.041 and p-0.019 respectively). The predominant phylogenetic group in our isolates is B2 (45.4%) followed by D (12.4%). Our results showed that 9.3% of isolates have an unknown phylogroup which shows a significant association with astA gene (p-0.008). We have as well found a significant association between B2 and three virulence genes namely pap, hlyA and invE (p-0.002, p-0.001, p-0.025 respectively); B1 and pap, hlyA genes (p-0.049 and p-0.021 respectively); E and afa gene (p-0.024). Conclusion: Certain virulence factors have been shown to be potential targets for drug design and therapeutic pathways in order to deal with the antimicrobial resistance problem enhanced by antibiotic therapy.

Anti-Toxoplasma gondii Activity of Curcumin: A Systematic Review of Pre-clinical Studies
Objective: This study has focused on anti-Toxoplasma gondii activity of curcumin. Methods: In this systematic review, anti-parasitic activity of Curcuma longa on Toxoplasma gondii was assessed. Therefore, several databases, including PubMed, Scopus, Web of Science, Embase and Google Scholar, were searched from 2010 to 2020. Results: Of the 2200 papers retrieved between 2010 and 2020, six articles were reliable and were scrutinized. In 2 in vitro studies, the most used strain was the RH strain of Toxoplasma gondii, whereas among 4 in vivo studies, RH strain was found in 2 (50%) studies, Me49 strain in 1(25%) study, and RH and Me49 strain in 1 (25%) study. In four in vivo studies, the most used animal model was BALB/c, and Swiss albino was found in 1 study (25%) and Albino rats in 1 study (25%). Conclusion: Curcumin and nanoparticles formulated with curcumin are new and useful agents for the treatment of parasitic diseases and reduction of drug resistance. The success of this therapeutic approach stems from the specific action of Curcuma longa against parasites and pathogens.

High Prevalence of Panton-valentine Leukocidin-encoding Genes in Methicillin-resistant Staphylococcus aureus Isolated from Inpatients with Invasive Infections at a University Hospital in Southern Brazil

Objectives: The present study aimed to investigate the prevalence of PVL-encoding genes in S. aureus isolated from clinical samples of inpatients with invasive infections in a teaching hospital in Southern Brazil. Furthermore, phenotypic and genotypic characteristics of bacterial isolates were analyzed. Methods: A total of 98 S. aureus isolates recovered from different body sites were characterized according to their antimicrobial susceptibility profile, methicillin-resistance and SCCmec typing, genetic relatedness and occurrence of virulence-encoding genes, such as icaA, lukS-PV/lukF-PV, and tst. Results: Sixty-eight (69.4%) isolates were classified as methicillin-resistant, and among them, four (5.9%) did not harbor the mecA gene. The mecA-harboring methicillin-resistant S. aureus (MRSA) isolates were grouped into SCCmec types I (6.3%), II (64.1%), III (6.3%), IV (15.6%), V (4.7%), and VI (1.6%). One isolate (1.6%) was classified as non-typeable (NT). Seventy isolates (71.4%) were classified as multidrug-resistant. The overall prevalence of virulence-encoding genes was as follows: icaA, 99.0%; tst, 27.5%; and lukS-PV/lukF-PV, 50.0%. The presence of tst gene was significantly higher (p < 0.001) in methicillin-susceptible S. aureus (MSSA) compared to MRSA isolates. Conclusion: The present study reports a high prevalence of multidrug-resistant S. aureus harboring lukS-PV/lukF-PV and tst genes in invasive infections. The continuous monitoring of the antimicrobial susceptibility profile and virulence of S. aureus is an important measure for the control of infections caused by this bacterium.

Molecular Docking Study of Potential Antimicrobial Photodynamic Therapy as a Potent Inhibitor of SARS-CoV-2 Main Protease: An In silico Insight

Recently, antimicrobial photodynamic therapy (aPDT) using safe and cost-effective photosensitizers has been introduced as a valuable therapy for the eradication of microbial infections. Objective: This in silico study aimed to investigate the potential of aPDT against SARS-CoV-2 main protease (MPro). Methods: In this study, to evaluate possible inhibitors of SARS-CoV-2 during aPDT, a computational model of the SARS-CoV-2 MPro was constructed in complex with emodin, resveratrol, pterin, and hypericin as the natural photosensitizers. Results: According to the molecular docking analysis of protein-ligand complexes, emodin and resveratrol with a high affinity for SARS-CoV-2 MPro showed binding affinity -7.65 and -6.81 kcal/mol, respectively. All natural photosensitizers with ligand efficiency less than 0.3 fulfilled all the criteria of Lipinski’s, Veber’s, and Pfizer’s rules, except hypericin. Also, the results of molecular dynamic simulation confirmed the stability of the SARS-CoV-2 MPro and inhibitor complexes. Conclusion: As the results showed, emodin, resveratrol, and pterin could efficiently interact with the MPro of SARS CoV-2. It can be concluded that aPDT using these natural photosensitizers may be considered a potential SARS-CoV-2 MPro inhibitor to control COVID-19.

Resistance Pattern in Mostly Gram-negative Bacteria Causing Urinary Tract Infections
Purpose: The antimicrobial prescription in urinary tract infections (UTI) is driven by local data on its pathogenic spectrum and the resistance pattern exhibited by the disease-causing pathogens. We aimed to determine the bacteriological diversity of UTI causing pathogens and antimicrobial resistance in mostly gram-negative bacteria. Methods: This retrospective hospital-based cross-sectional study analyzed the culture and sensitivity reports of urine samples from a referral centre in the Aljouf region of Saudi Arabia. All the antibiograms from January 1, 2020, to December 31st 2020, were included. The bacterial identification and antimicrobial testing were carried out by the BD Phoenix system (BD Diagnostics, Sparks, MD, USA). Antimicrobial testing was performed as per the Clinical and Laboratory Standard Institute recommendations. Frequencies of multidrug- and extensive drug resistance were calculated. Results: Of the 1334 non-duplicate urine samples received, 422 (31.6%) bacterial growths were observed. Of these, 383 (90.8%) and 39 (9.2%) were gram-negative and gram-positive bacterial isolations, respectively. E. coli 161 (38.1%), K. pneumoniae 97 (23.0%), and E. faecalis 18 (4.3%) were frequent aetiologies of UTI. 309 (80.7%) of gram-negative bacteria were multidrug-resistant including 88 (23.0%) extensively drug-resistant. Overall, a resistance rate of > 55 % to 1st through 4th generation cephalosporins was observed except for cefoxitin (43.7%). A resistance rate of 37.6% was observed towards carbapenems, with the lowest rate (34.0%) to meropenem. Conclusion: Multi-drug resistant gram-negative bacteria dominate the pathogenic spectrum of UTI in the region. A high resistance rate to cephalosporins and carbapenems exists in gram-negative organisms, causing UTI.

Biofilm Formation Status in ESBL-Producing Bacteria Recovered from Clinical Specimens of Patients: A Systematic Review and Meta-Analysis
Authors: Masoud Keikha and Mohsen KarbalaeiBackground: Recently, the emergence and spread of extended-spectrum beta-lactamase (ESBL) bacteria have become a global health concern. In addition, the ability to form biofilm due to less impermeability to antibiotics and the horizontal transformation (conjugation) of genes involved in antibiotic resistance have exacerbated the concerns. With a comprehensive meta-analysis, this study evaluated the potential relationship between ESBL and biofilm formation. Methods: A literature search was performed using global databases, such as PubMed and Scopus, up to November 2021. We retrieved all relevant documents and selected eligible articles based on inclusion criteria. Finally, the potential association between the biofilm formation capacity and resistance of ESBL-producing bacteria was measured with an odds ratio and a 95% confidence interval. Results: In the present study, 17 articles, including 2,069 Gram-negative isolates, were considered as eligible. The prevalence of biofilm formation in all clinical isolates of ESBL and non-ESBL pathogens was 72.4% (95% CI: 60.7-81.6) and 40.5% (95% CI: 30.2-51.8), respectively. Our results showed a positive relationship between the ability for biofilm formation and conferring antibiotic resistance in ESBL-producing bacteria (OR: 3.35; 95% CI: 1.67-6.74; p-value: 0.001). Conclusion: In general, we showed the rate of biofilm formation to be significantly higher in ESBLproducing strains. Given the current results, the updated therapeutic guidelines should consider the role of biofilm production for optimal therapy, treatment course, and clinical outcomes rather than the recommendation of antimicrobial agents by focusing on the results of the antibiotic susceptibility test.

Cryptosporidiosis in Yellow Nail Syndrome – A Rare Case Report

Association of Yellow Nail Syndrome (YNS) and Cryptosporidiosis is rare and has not been reported till date. Immunity can also be affected in this case of YNS is associated with autoimmune disorders. Case Presentation: Here, we describe a case of persistent diarrhea in an 8 month old YNS patient. Modified Ziehl-Neelsen staining and Saffranine-Methylene blue revealed oocyts of Cryptosporidium species. Following appropriate treatment, the patient’s symptoms improved and the patient was discharged in a hemodynamically stable condition. Discussion: Cryptosporidiosis is a significant cause of morbidity and mortality in immunocompromised patients. YNS per se as well as treatment including steroids leads to immunosuppression in individuals making them susceptible host for opportunistic infections like Cryptosporidiosis. Conclusion: Clinicians should be aware of the condition and screen for Cryptosporidiosis in any immunocompromised patients with diarrheal symptoms, as parasitic infection like this are opportunistic in them.

Disseminated Tuberculosis in an Immunocompetent State: A Case Report

Clinical manifestations of TB are broad and sometimes very challenging for clinicians to diagnose early. Tuberculous psoas abscess was generally secondary to spinal tuberculosis or direct extension from adjacent structures in immunocompromised individuals, but tuberculous psoas abscess in the immunocompetent state is very infrequent. In addition, pancytopenia and new onset neck swelling simultaneously make this presentation a very unusual clinical entity in tuberculosis. Case Presentation: We now present a case of a 21-years-old, unmarried, otherwise healthy girl presented with fever, lower abdominal pain and weight loss for two months. She also noticed painless neck swelling for 15 days. She later had a tuberculous left sided psoas abscess with pancytopenia and a cold abscess on the left side of the neck with no sign of any other apparent focus, according to the evidence. Diagnosis of disseminated TB without lung involvement was established and ATT was started. The outcome was successful on follow up. Conclusion: Among the broad spectrum of atypical manifestations of TB, this case report draws attention to its rarity, diagnostic challenge and awareness of the clinical spectrum, especially in developing countries.

The Determinants of Treatment Delay and Quality of Life among the Newly Diagnosed Pulmonary Tuberculosis Patients in Northeastern Thailand: A Cross-Sectional Study
More LessBackground/Objective: Delays in diagnosing pulmonary tuberculosis (PTB) are linked to financial difficulties, employment limits, dependency, and symptomatic treatment, all of which have an impact on the patient's quality of life. Patients' psychological, economic, and social well-being are also harmed by delayed therapy. The goal of this research was to evaluate sociodemographic characteristics and quality of life in new pulmonary TB patients and determine the associated factors with delayed TB diagnosis. Methods: This was a cross-sectional study conducted in the Choke Chai Community Hospital, in Northeastern Thailand between 2016 and 2018. The information related to the patient’s medical history and laboratory tests were gathered from 332 newly diagnosed tuberculosis patients at the hospital's tuberculosis clinic; of those 15 died and were excluded from the final analysis. Data were analyzed using SPSS version 17.0. Results: The cases included new cases (94%) and those returning to receive treatment after discontinuation of treatment, relapse, or recurrence of the disease (6%). The sample consisted of 68.7% males and 31.3% females. The average age was 52.7 years (S.D = 15.64). The majority of patients were married (60.5%), employed (68.6%), received a low income (66.6%), had a history of chronic diseases (73%), drank alcohol (31.7%), and smoked (85.7%). Approximately half of PTB experienced treatment delay and had a low quality of life (QoL) (46.6%). The treatment delay was associated with age > 50 years (p <0.05), low self-care (p <0.05), long distance to facility (p <0.05), unavailable caretaker to treat (p <0.05), no caregiver in family (p <0.05), high expenses (p <0.01), and lack of information (p <0.001). Conclusion: It appeared that the patients' health-related quality of life could be deteriorated as a result of pulmonary tuberculosis. Treatment delays can be significantly reduced by changing the understanding of family caregivers, increasing awareness, providing adequate support for patients, and guaranteeing early diagnosis and treatment by implementing an efficient surveillance system.

Detection of Antibodies against Toxoplasma from Human Serum Sample using ELISA

Clinical features range from mildfebrile illness and lymphadenopathy in the immunocompetent host to encephalitisin the immunosuppressed host (E.g. HIV infected patients). Congenital infection can also occur. For effective control and treatment of toxoplasmosis, accurate detection of T. gondii infection is important. Objective: In this study, ELISA detecting anti-toxoplasma antibodies IgM and IgG has been used for diagnosing Toxoplasmosis in patients. Methods: Enzyme-linked immunosorbentassay (ELISA) was carried out in serum samples collected from 40 patients to detect anti-toxoplasma IgG and IgM antibodies as a part of work up in suspected cases. Relevant clinical history was also taken. Results: Of the total 40 samples taken, only one sample came positive for IgM and 9 came positive for IgG antibody. All patients who were seropositivefor T. gondii antibodies had HIV infection. Five patients were in the age group between 30 to 40 years. Conclusion: From the limited data available in this study, it may be recommended to screen for T. gondii antibodies in HIV patients.

Interplay between SARS-CoV-2 and Cancer: Plausible Risk Factors, Cellular Immune Responses, Cancer Directed Therapy- Current Challenges

According to new evidence, the COVID-19 pandemic has the potential to aggravate pre-existing inequities, particularly among cancer patients. The COVID-19 pandemic has had a disproportionately negative impact on cancer patients. The unfavorable outcomes in cancer patients who contract COVID-19, the impact of the COVID-19 pandemic on cancer care delivery, and the significant disruption of cancer research are all examples of this consequence. On the other hand, patients with cancer are a diverse group, and new research has identified characteristics that allow for risk categorization of cancer patients to optimize care. Variable access to telemedicine, timely diagnosis, and treatment access are all possible drivers of unequal cancer survival as a result of the epidemic. Despite oncology associations presenting guidelines on cancer care during the pandemic, the magnitude of potential therapy advantages, therapeutic purpose, and access to care all play a role in prioritizing cancer medicines. This review focuses on evidence related to COVID-19 and cancer, such as the molecular interactions between the two diseases and practical therapeutic suggestions for cancer patients during the pandemic. We also explore the pandemic's possible long-term influence on cancer care due to its negative impact on cancer research, as well as biological discoveries from the cancer research community that could aid in the development of novel therapeutics for all COVID-19 patients.
